HO transformer question

I have an HO transformer item # 46605A that is two years old. I thought I broke it in correctly, but while operating, my train is either on fast or off. No graduation of speed. Can I resolve this?

I've had these apart, and I pretty well understand what makes 'em tick. 

How does it work in reverse?

How do you cause it to go "fast"?

How do you cause it to turn "off"?
